Suggest Treatment For Sore Near Natal Cleft Of Buttocks

Question: I have had a very sore butt crack area for almost three weeks. It continues to get worse. It started near the top of my crack with some irritation and after a week of walking and biking on vacation it became very sore. I eventually got around to looking at it and it is a red irritated mess on both sides out my crack and there is a deep split at the deepest part of my crack. About 4 days ago I felt a little hardness there so I squeezed (though it was very painful), and a bunch of foul smelling white blood streaked pus came out. I did get some relief, but that was short lived. I assumed this was a cyst of some kind. I am worried about all the redness and some raised skin in the area, as well. Also, I have a couple cuts closer to my scrotum that sting a little bit, but they are not terrible. The whole area has been moist from puss and also ointment.
I have had a history of a very bad fungal infection in the same area about three years ago, but there was no puss. Otherwise, this feels fairly similar though it does not yet involve my actual anus, which is not painful at all during wiping or bowel movements. I had previously noted some itching near my tail bone with no other associated symptoms and now I do feel more itching in the area. Well over 2 weeks of continually worsening pain.
I have been applying lotrimin ultra for two full days now. That gives relief in that it seems to lubricate, but no huge changes. Not sure if I should stick with the lotrimin or if it is something else? I tried to get a couple pictures, but it is hard with the hair and angle. Please help! This is not very fun.

Thank you for the query.

From your description and from the pictures I am almost sure that you have pilonidal cyst. This cyst is caused by an ingrown hair. Such hair irritates tissues causing cyst creation. This appears with kind of tiny dent in the skin.
Sooner or alter, such cyst becomes infected resulting with redness, pain, swelling and pus.

There is no other treatment than surgical removal of whole cyst. Please consult general surgeon. Without this procedure, this can get worse.

This was my big fear! This is miserable. And the small lesions on the other side of my anus near the scrotum? I assumed this was either from being wet all the time or more fungus. Do you have any thoughts?
Also, because of the raised skin around the split I figure more likely to have fungus as well. After my last bout with fungus I am paranoid. Also not fun. Can all of the bumps and redness be a result of the cyst? Thank you.

Fungal infection seems to be present as well.

Detailed Answer:
Actually the cyst can have few openings (usually it does). So there can be more of kind of bumps with a tiny hole.
However the cyst does not reach the scrotum or does not passe on the other side of the rectum. So most likely some part of the redness is due to fungal infection as well.
When you have a cyst, and its leaking, the environment for the fungi is excellent.
For sure you have cyst. And for sure it has to be removed.
Redness can be from fungal infection and the other lesions as well. But this should be confirmed by dermatologist.
