Question: I recently developed a sore under my foreskin bout 4 days ago. It does not ooze, nor hurt. In the past i have had smegma build up. Pictures are available.

The lesion on the penile shaft and below foreskin appears to be an ulcer.
Is the ulcer painless?
If painless, a test for syphilis should be done since syphilis is characterized by a painless ulcer on the external genitalia.
Avoid sexual contact till the cause of ulcer is found and proper treatment is instituted.

A moderate degree of pain associated with ulcer can be seen in sexually transmitted disease (STD) other than syphilis.
The ulcer of syphilis is painless, however, ulcers associated with Hemophilus ducreyi are painful.
A consultation with a dermatologist is required for clinical assessment and relevant investigations.

Could be balanitis, cause needs to be known

Fungal infection is usually associated with profuse itching and white or green colored discharge.
The lesion is balanitis, however, the underlying cause of balanitis has to be found out to treat the ulcer.
