Brief Answer:
It is an advance disease.

Detailed Answer:
hi dear,
After reading it question and after looking at the CT scan report, I may say that the disease is really advance. there is a large tumor in liver and also remaining liver is not healthy as it is cirrhotic. so the liver is not able to work properly and hence there is jaundice.
the blood supply to liver is also not proper as there is thrombosis in portal vein.
please upload àll reports so that I can arrive at a conclusion. I need biopsy, CBC and LFT report.
it is right that in presence of jaundice chemotherapy is not advisable, because it may worsen the liver failure.
if the diagnosis is hepatocellular carcinoma and Condition of patient allows we may start a tablet sorafenib.
keep yourself strong in such a difficult time.
my best wishes are with you.
please feel free to ask further.

Brief Answer:
Improve liver function first.

Detailed Answer:
hi dear,
the AFP is very high more than 5000. it is in favour of hepatocellular cancer.
but we can't start treatment or chemotherapy without biopsy.
you did not send the report of liver function tests. i would like to know what is the level of bilirubin and albumin.
also important is to know the general condition of patient called as performance status.
his liver is small, it is due to cirrhosis and so not functioning properly.
if liver doesn't function properly it is difficult to start any chemotherapy because chemotherapy iabtoxic to liver and it may further decrease the function of liver.
so first we should start the treatment of cirrhosis.
once bilirubin come down, we can then give chemotherapy.
we all are with you in this difficult situation.
feel free to ask further.

Brief Answer:
Please start treatment for cirrhosis.

Detailed Answer:
hi.
at a bilirubin of 8.3 we can't start chemotherapy. may be this is the reason they have not advised biopsy.
so please remain in contact with the hepatologist. first he should get treatment for cirrhosis. once liver recovers, bilirubin will go down. then we will do biopsy and then the medicine for probable cancer.
hope he will be Healthy soon.
my prayers are with him.
