Question : on-going issue that is entirely new to me (62 yr old male, excellent health otherwise)
Un-predictable hic-up's affiliated with stomach discomfort alleviated by eating, but soon returns. All began six weeks ago after a mild flu; truly un-comortable when supine especially bedtime; I have imagined it might be related to constipation, but I continue (as for 30 years) to have wheat XXXXXXX every day on cereal or in protein shake. I visited the surgeon who did my clonoscopy but he did not recommend a repeat.

The issue dis-appeared for a few weeks but returned yesterday and after about an hour of midnight discomfort was relieved by NaCo2

Hi.
Thanks for your query and an elucidate history.
To recapitulate: Male/62 - noted medical history and current medications - unpredictable hiccough/ hic-ups affiliated with stomach discomfort - alleviated by eating - returns - started 6 weeks ago after a mild flu - uncomfortable at supine position at bedtime - ? constipation - visited surgeon - disappeared for a few weeks to reappear yesterday - after an hour of midnight discomfort relieved by Soda-bicarb - noted the medications tried ....

My thoughts:
Hiccough is usually caused by the irritation of the diaphragm and the possible causes are:
- Irritation of the phrenic nerve that supplied the muscles of diaphragm.
- Pathology underneath like that if liver, stomach and so on.
- Pathology above like inflammation of the pleura, lungs or even the esophagus or mediastinal contents causing irritation of the nerve in the chest or neck.
- general diseases like where urea and creatinine rise.

Since the problem is for about 6 weeks and appeared after a gap of few weeks I would recommend to undergo clinical evaluation, examination and investigations to get to the possible cause and get a proper treatment.
- Tests of blood: CBC, urea, creatinine, liver function tests and as guided by clinical evaluation.
- Preliminary X-ray of the chest and neck and confirmation by CT scan if indicated so.
- Ultrasonography of the abdomen and CT scan as indicated so .
- Any other tests as indicated.

These will help to get a proper diagnosis and get a proper treatment accordingly.
