Question : Hello Doctor,
I rolled over the bed over 4 weeks back and I now undrstand that I had a penal fracture.
I ejaculated and there was lot of blood all around.But absolutely no pain.Now the penis is bent down and one part of the head is purplish.i wake up at night when there is erections which is swollen and again a bent in penis and erection is very mild/.I have lots of tinglings and sometimes shooting oain which lasts for a second.i will go back to XXXXXXX in another 4 days where i would be consulting a urologist.i have been taking ibprofen on and off.I have lot of tingling sometimes.Also i have urination problems like small urination.and I am feeling very suicidal.

Thanks for your query,based on the facts that you have posted it appears that you have sustained injury to your Penis mostly when it might be erected during sleep and you rolled up over bed.It is likely that the injury has extended from Tunica Albugenia a sheath that cover vascular erectile tissue into urethra causing bleeding during ejaculation.

Please consult qualified Urologist for clinical examination once you are back in XXXXXXX and get following basic tests done to confirm the diagnosis.
1) Ultrasound scanning of Penis .
2) Colour Doppler study of Penis.

There is nothing to be worried ,it will recover to normal after suturing the rent in Tunica Albugenia .In the mean while continue taking pain killers.

i think the bleeding was from outside and not from the tip of the penis..Also i think was masturbating.I had no pain whatsoever but lot of blood all over.Also it has lead to eryctyle dysfunction and a bent down and i think deformation of penis.Will these be permanent ?Also is it a penile fracture?will i have ever able to have normal funtions?I am feeling lost and feel like my life had ended.I have no kids yet.

Thanks for follow up.
It is no doubt that you have sustained injury (Fracture ) of the penis that is causing loss of erection and bending down of penis.

Do not worry this will recover completely and you will get normal hard erection after the the rent in Tunica Albugenia is sutured .so that you will have normal sexual life .

In the mean while avoid any sexual activity like masturbation or intercourse till you undergo surgery.

Please consult Urologist immediately after coming back to XXXXXXX for evaluation and further treatment.

I had no pain at all during the incident.is it still it is of great embarassment to me n my family
There has been no blood after the incident.there is some swelling though.how many ubugesic plus can i take a day.also could u please let know how long will this surgery take and to recover.also could you please let me know ur details so that i can come n meet you in XXXXXXX
First of all thanks for you time sir.I will correct that statement.I had lot of blood soon with the incident but later i had no signs of blood anyday.I do have some swelling.and burning at the tip of my penis after I urinate.I thought this was some uti and took levoday after an advise from a doctor.the burning subsided but again started when i didnt take for 2 times now.
How many Ibugesic plus can I take a day?Also could you please let me know how complicated is this surgery and know how long will this surgery take and to recover?

Your symptoms are not due to UTI.but due to injury to your penis.

You can take Ibugesic for pain twice a day.

Surgery is not complicated and will take one week for full recovery.However you can continue your routine work except sexual activities.

One last question along sir...is the tingling sensation along the tip and sometimes elsewhere also due to the same?

Thanks a lot sir.It really helped me reassure things.Does it require 1 week of hospitilisation?
There is no constant pain for me but occassional feeling of burning and sudden weird feeling of lost it.(i dont know how to explain this).
But till then I hope this does not emerge into an emergency situation.
I will visit you on Monday as soon as I land.Will the surgery also happen at VIMS?
I am feeling that my urine has reduced.(Also I have reduced intake of water now).
I also have a burning sensation at the tip of my penis sometimes and hence thought UTI.
I am sure this is a very shameful incident and totally not common so I thought it is very complicated.Am extremely thankfull for your help sir.Sorry for bothering with so many questions.

I can understand your anxiety,

Ideally this should be treated immediately but you being out of country and as there is no swelling of penis or bruising of the penis it can be treated little late .

The surgery will be decided by me after clinical examination and confirmation by Ultrasound Scanning and Colour Doppler study of penis and will be done in VIMS Hospital .

The tingling and pain at the tip of penis may be due to injury itself however possibility of UTI has to be excluded by routine urine test and urine culture.
