Brief Answer:
Examine by a gastroenterologist ,urologist or general surgeon.

Detailed Answer:
Hi Dear User ! Thank you for using HCM.
I read your questions and I understand your concern .

You should go to a local doctor ( urologist, gastrologist or general surgeon ) to do a physical examination and to take some topical medications that will help you.
The symptoms you described tell me about hemorhoids problem. The local doctor will see the area and will do an anoscopy( for internal hemorhoids ) to confirm the diagnosis or if it's necessary some other examinations.

Meanwhile follow these advices :
- drink planty of water (liquids,, 6-8 glasses of water /day )
- increase fiber intake in your diet ( in this way you avoid constipation that makes the symptoms worse ).Gradually increase insoluble fiber to 25-30g/day (raw fruit, vegetables, cereals or supplement)
-Avoid straining and reading while on toilet ,constipation .
- losing weight and do excercises that don't increase abdominal pressure .
Bycicle is not good for your condition cause of the body posture .
- Good perianal hygiene ( wash with warm water )

Here are some topical medication your local doctor will advice and that will help : local anaesthetic, steroids(Anodan-HC 0.5%/0.5%, Anugesic HC..),Lidocaine ointment 5% will reduce
the pain . .Topical nitroglycerine 0.2%ointment .
According to the examinations You will discuss with your local doctor about other options (small surgery).
Hope I have answered your questions!
If you have any further clarifications I will be happy to help ,if not you can close the discussion .
Take Care !
Dr Balliu.
