Usually such bulge is a hernia of the umbilicus. It may be painful or painless. Can come and go. Especially when you lying down, should be less visible. Sometimes its easy to push it back to the abdomen.
The best way to diagnose it, is abdominal ultrasound. If its hernia, surgery should be considered.
