Brief Answer:
Need to do a 2D Echo

Detailed Answer:
Hi,
Welcome.
This is Dr Sameer, cardiologist.
Pedal edema(swelling of foot) is a sign of fluid congestion in body. I can understand that you also have a pacemaker implanted.
So i will advice you to get a 2D Echo done first to evaluate the current status whose dysfunction can cause fluid congestion in body.
Get back after doing the echo.
Thanks & take care
