not sure how long this has been going on, but at this extreme
only 3-4 days

Thanks for your query, albeit short and specific.
To recapitulate: Male/77 - swollen scrotum - larger than soft ball - not sure of duration but at its extreme for 3-4 days - wants to know how serious is this....

Please give additional information:
- Is there pain, redness, tenderness?
- Was there any history of severe pain in the beginning 4 days ago that has reduced?
- Any history of hernia?

Scrotal swelling that you have noticed 3-4 days ago and of the size of a soft ball must be taken seriously till the proper diagnosis is done and this is possible by the following:
Clinical evaluation, examination by a General Surgeon or Urologist.
More important is Ultrasonography and color doppler of the scrotum and abdomen to get the proper diagnosis.
Only a proper diagnosis can lead to a proper treatment.
Tests of blood and urine also needed.
