Brief Answer:
If rapid spread, fever, appropriate to see emergency physician.

Detailed Answer:
Hi and thanks for the query,

With this spread, this requires urgent attention. This could imply a rapid spread of the infecting germ. Especially, it is important to know if she has started developing any fever or malaise. If yes, it should be a sign of the condition becoming a bit more serious.

If the rate of spread is relatively very fast, I would suggest you visit the emergency room. There are some germs that with excessive spread in blood can rapidly lead to sepsis and at times a septic shock. This is rare but a possibility if the rate of spread of inflammation is really great.

Doxycycline is a good antibiotic. However, the fact that even with it the spread is still fast, injectable antibiotics could be more appropriate accompanied by some drugs to fight against inflammation.

The change from oral drugs to injections shall be made by your doctor based on his clinical judgement. Simple blood tests like the C reactive protein and Erythrocyte sedimentation rate that give an idea with regards to the degree of inflammation could be easy to do and give an idea when she sees the doctor, guiding the right action.

To be safe, i suggest a review by the emergency room physician.

Thanks and kind regards as I wish you the best of health.
