I am dr XXXXXXX Sultan and will be answering your query. I have read and understood your health concerns...
Is she having diabetes or hypertension?
For his long she is suffering from ankle swelling?
Is she having fever?
Swelling of ankles with redness and painful movements shows joint problem. But I think it is more appropriate if you send me the photographs of it, if they are red and swelled up right now, so that it can be correlated properly.
If I am her attending physician , I would definitely get her x ray of both ankle joints along with full blood count( FBC) , ESR and CRP. RA factor testing is also appropriate. After then I will be able to proceed.
Cardiac surgery had nothing to for with it.
Meanwhile stay calm and use acetaminophen to relieve pain..

 My wife's ankles are not swollen at the moment.
She isn't diabetic, she has hypertension but the drugs she is taking control it. She doesn't have a fever. The ankle problem comes and goes and there doesn't seem to be a particular cause. Walking seems to help which suggests that her joints are ok.
She has regular blood tests since her open heart surgery was only a year ago.
no need of any intervention right now

There is no need to intervene right now. If they become red and swollen again then you can see orthopedic surgeon..
Meanwhile continue with your medicines and Engage in healthy physical activity.
