
Suggest Treatment For Symptoms Of GERD



I have been experiencing a crackling and gurgling from my chest/heart over the past few days. I noticed it especially at night but also during the day. My RHR is 55-65 BPM over the last 7 days. I have been running and doing yoga and have had no chest pain or short of breath. I do notice that if i have eaten a big meal, i have trouble running and feel completely out of breath but otherwise on a day where i run in the morning or have not had a heavy meal, my running feels great! My doctor had told me that i had mild regurgitation and was not worried about it a year ago or so, i have not seen him since. I have no symptoms other than this gurgling noise with each breathe that has kept me up at night and thus i am only
Sleeping 1-2 hours per day - i am traveling in Asia on work and normally live in the US so a combination of jet lag and this noise is something i am struggling with - please give me your advice.

I carefully passed through your question and would explain that your symptoms seem to be related to gastro-esophageal reflux.
Based on your description and your past medical history (for mild reflux), I would explain that your reflux seem to be exacerbated.
But, there is nothing to be concerned of.
I would exclude any cardiac disorders, because your symptoms are not typical of any cardiac disorders.
So relax and don't worry about it.
Coming to this point, I would recommend taking an anti-acid or PPI (omeprazole, pantoprazole, etc.).
Metoclopramide 10mg at bedtime can help reduce your troubles during sleep.
I would also recommend some tips to help you improve your situation:
- avoid spicy food
- avoid soda drinks
- eat frequently during the day in small portions and try to avoid large meals.
If the situation persists, I recommend consulting with your attending physician and performing a fibrogastroscopy.

1. I woke up to chest discomfort towards middle, repeated when pushing on sternum and feels like a burning feeling
2. When i sleep to my left, i feel discomfort and slight pressure but no shortage of breathe
3. My heartbeat is 66 but its a strong beat that interferes wirh my sleeping, it seems like my whole upper body is beating
4. I certainly hear a lot of sound from my upper stomach especially acidic gurgling but i also hear something from chest that coincides with the heart beating
5. Could you let me know what type of symptoms i would see from an aortic valve malfunction - i realize i tend to worry so this has been my biggest worry, that my valve will fail due to the aortic regurgitation especially when i keep hearing the gurgling
5. Is there any way we can speak by phone at some point? I have used the app to a considerate amount and given my concerns this would help me

I carefully reviewed your uploaded HR images and would like to explain that they seem to be perfectly normal for a healthy and normal individual.
Regarding your symptoms, I would explain that they are typical of gastro-esophageal regurgitations.
Regarding the sound in your upper stomach, coinciding with your heart beating, I would explain that they could be irradiating sound from the heart to the diaphragm walls, which are nearby to the stomach and the heart. Another possible cause that may lead to these symptoms would be a hiatal hernia. But even this is not a concerning medical condition.
They are not indicative of aortic valve malfunction. I would like to explain that in such case you would not be able to exercise normally even when eating small meals, you would have shortness of breath, fatigue and maybe leg swelling. You can not feel the sound of aortic regurgitation.
But, you have none of these symptoms. So relax and don't worry about it.
I recommend follow my tips and recommendations (about eating, antiacid and metoclopramide).
These are OTC drugs, which do not need medical prescription.
If your situation improves by following these recommendations, it would be a strong indication of gastro-esophageal reflux or hiatal hernia underlying your symptoms.
Otherwise, I would recommend consulting with your doctor for a physical exam and some tests:
- chest X ray study (this test would also help investigate for hiatal hernia)
- a cardiac ultrasound
- a fibrogastroscopy
- some other tests (complete blood count for anemia, thyroid hormone levels for thyroid dysfunction).
