Question: MY daughter (19) has been having shortness of breath and her esophagus burns and when she swallows there is pain about 6 seconds after she swallows but the pain is down around her chest not in the throat. The pain starts at the chest then moves around the back. She hasn't been able to sleep because of the back pain. Last week she was taking her vitamins at night before she goes to bed. She didn't take enough water with them. Then went to bed. She also lifted a heavy object and carried it around at chest height didn't know if you needed to know that.

I have gone through your details and i appreciate your concerns.

Most likely your daughter is suffering from severe acid reflux problem.Acid is produce by parietal cell of stomach,As soon as we swallow something the brain stimulates the parietal cell s to secrete acid.In between the esophagus and stomach there is a sphincter like muscle which seperates both.If the muscle is loose the acid produced in stomach regurgitates back into esophagus causing severe inflammation and irritation of esphageal mucosa.This inflammation is responsible for severe pain in thoracic region(chest and esophagus),Acid leaving her stomach is causing severe heartburns.When mucosa of esophagus is irritated it secrtes large amount of mucous causing shortness of breath.

Initially few tests are recommended-
1)Ambulatory 24 hour Ph monitoring-Ambulatory acid (pH) probe tests use a device to measure acid for 24 hours. The device identifies when, and for how long, stomach acid regurgitates into your esophagus.

2) Endoscopy is a way to visually examine the inside of your esophagus and stomach. During endoscopy,a thin, flexible tube equipped with a light and camera (endoscope) is inserted into esophagus.This also help to find out some serious complications like XXXXXXX esophagus.

3)Esophageal manometry measures movement and pressure in the esophagus.

Treatment suggestion-
Give her Omeprazole 40 mg once daily(it reduces formation of acid).
Give OTC antacid to her 5 minutes before lunch dinner and breakfast to neutralize excess acid.
Get a prescription for Prokinetic drug like Levosulpiride(to prevent acid reflux).
Ask her not to take spicy or oily food.
No aerated beverages,smoking or alcohol.
No coffee or tea.
No fast food like pizza burger etc.
Drink lots of yoghurt and curd.
Take 2 cup of chilled milk twice a day.
Avoid juices of all citrus fruits like orange grape etc.
Raise your head end while sleeping to prevent acid regurgitation in esophagus.
Drink plenty of water.
Avoid taking water between meals.

Follow up with reports of investigations suggested.

Hi again,
Yes acid reducer plus strict dietary changes will help her to heal herself but it will take time and strict adherence to specific diet as mentioned above by me.

Vitamin or water has nothing to do with it.
She is suffering from acid reflux which is a disease in itself.
