Suggest Treatment For Symptoms Of IBS While Having Anxiety

Question: Dear doctor - I suffer from anticipatory anxiety and phobia of meetings. I am on the following medication:
Sulpitac 100 in the morning
Opiprol 50 in the morning
Inderal la 20 in the morning
Parageb od 75 in the morning
Tynept 12.5 mor/ eve
Quetiapine sr 100 in the night
Sizopin 25 in the night
Zapiz half of 0.25 in the night
With these combination of medicines, my anxiety is better but my ibs ( I suffer from ibs also) has come back big time ( churning, bloating, pain, burning sensation in the stomach with slight constipation). Went to my doctor today, he has told me to add lesuride 25 mor/eve ( I was taking lesuride but stopped it some three days ago as I was not getting any respite from my stomach problem) and told me to take parageb od 75 in the night instead of morning. I need advice from you on the following:
What is causing ibs
After taking tynept and lesuride, which are supposed to control ibs, they are not helping me
What alteration according to you I should do in my medicine to bring my ibs under control as it has started affecting my daily life and causing hindrance at work

Hi, thanks for asking again

- IBS symptoms came back due to relapse of some anxiety symptoms. IBS is related to anxiety and the symptoms like churning, bloating, pain, burning epigastrium etc are part of anxiety only. Best treatment apart from medicines in such symptoms is to ignore the symptoms. Awareness of milder symptoms related to abdomen results in unconscious checking if these are gone or not and this increases anxiety. Anxiety further aggravates IBS symptoms. As your anxiety is relatively under control so with time these symptoms too, will go away.

- Lesuride is Levosulpiride and is related to Sulpitac. I don't think there is much advantage in taking both drugs together. Continue to take Sulpitac as prescribed or you can take 100 mg in morning only. If symptoms persist even then ask for dose reduction to 50 mg per day. Low doses of this drug help to control the negative symptoms and lift the mood.

- Apart from reducing the dose of Sulpitac (if needed) other option is to take Zapiz 0.25 (or if available 0.125) on SOS basis when ever you have excessive IBS symptoms. This will control episodes of increase in anxiety.

- Take food rich in proteins and complex carbs. Avoid fried food or foods containing saturated fats. Walnuts, salmon or sardine fish etc are rich source of Omega Fatty acids and help to control anxiety and also supplement the actions of drugs. Include these food items in your diet. Walnuts are rich in fibres too so will control IBS symptoms too.

I didn't take sulpitac and opiprol at all today and still my ibs persisted. Symptoms are severe and difficult to ignore.

Also was wondering if adding mirtazipine will help as it has helped me in the past.

Mirtazapine can help in controlling anxiety more effectively and that can control IBS symptoms. But the drug is known to increase weight and is not a very effective drug that controls IBS symptoms directly.

Don't stop taking Sulpitac if possible reduce the dose to 50 mg. This should help you. Continue to take Opiprol as advised.
