Question : Should I be concerned about this? It is less than 6 cm in size and located on the heel of my left foot. No pain or discomfort associated with this and it has been there for a good while.

If you are talking about this blackish brown lesion, then it is, I think 6 mm and not 6 cm in size. Only concern is that of developing a melanoma. If the lesion increases in size, is painful, develops irregular border or ulcerates then a biopsy is required. Do watch for these signs..
