Suggest Treatment For Symptoms Of Migraine

Question:
So today I had 5 dizzy spells lasting around 30min. I spoke with a doc late last night after speaking with you and he told me to take 2 baclofen and 2 lorazepam and that my migraine would be gone when I woke up in the morning. When I woke up my migraine was less but about an hour and a half later it came back fully and I started getting the blurred vision, nausea, light and sound sensitive and dizzy spells. I am on ondasterone for nausea and meclizine for dizziness. Now the dizzy spells are affecting my life, I am having trouble driving my car. I had to pull over my vehicle in the middle of a dizzy spell and sit for 20 min. I was afraid to drive. What do I do? What is wrong with me? I am afraid...

I carefully read your question and understand your concern.

Headaches with sensitivity to light and noises associated with nausea are typical for migraine. Migraine is a self limiting disease and does not affect your health, it is not dangerous in general but can be very devastating when you have an episode as it might prevent you from doing your daily activities sometimes.

Ondaseron is a good medication for the nausea so I would advide you to take it in case it is helpful.

As for migraine medications there are several options. One is the combination pills with acetaminophen/aspirine/cafeine (Exedrin migraine).
If the above tablets do not help than the most effective treatment is sumatriptan. This is the first like and best treatment for migraine. You will need to start it as soon as possible to have better effect.

Avoiding some food is helpful also, for example avoid chocolates, cheese and alcohol as some people get migraine headaches after using them.

if you have headaches more often than 3 days a week you might need to have some prophylactic treatment with oral tablets that can be taken daily for 3-6 months to reduce the frequency and intensity of the headaches.

Baclofen and Lorazepam are not in the recommendations for migraine headaches treatment. I would not use Lorazepam as it has many side effects and drowsiness is one of them and also it might cause dependence if used often.

If you are having a migraine episode and you feel you are not able to drive than you have done the right decision going aside. i know this might seem inconvenient as sometimes you need to go to the destinaiton but it is safer. Lorazepam might contribute to drowsiness, I do not know the time you got it but its effect can last 6-8 hours and if you tood 2 tablets of it the effect might go on a little longer.

if you have neurological symptoms like numbness, weakness in one leg or arm, or difficulty to speak or to swallow , or headaches last more than 72 hours, than you will need to see a doctor to check you and rule out other causes.

I have now had this migraine for 14 days and the dizzy spells for 7 days. This isn't just a one day thing. I asked this as a follow up to my other question but no one answered. So now I am sitting at day 14 with a migraine, I have been to the hospital on day 9 like my doctor told me due to vomiting and all my other side effects from migraine along with massive dizzy spells. But now dizzy spells are becoming more prone. Hospital gave me ondasterone and meclizine and 2 liters if IV fluid. I want to know what to do after 14 days? What is this? Why is this happening? What are these dizzy spells? And why didn't the hospital admit me when my doctor asked them too?

I understand the frustration and your concerns.

Migraine headaches should not go on for longer than 72 hours in an episode.
In your case you are having a very long episode to be simple migraine so you do need to have an MRI done to check for any causes that are not related to migraine.

There are other causes of headache with nausea other than migraine.
The three most important diagnosis to rule out is:
1. brain mass: MRI should help to rule this out
2. infection like meningitis and encephalitis: you will need to have some tests to rule these out from blood tests to sometimes tests of the spine fluids
3. hemorrage: MRI would help to rule this out and sometimes the best test for hemorrages might be CT scan.

Please set an appointment with your doctor. Your headaches are not behaving as a migraine headache behaves- it should not last longer than 72 hours and pain killers should have helped.
