Suggest Treatment For Tear Of Subscapularis Tendon

Question: Hi,
I had a work injury resulting in a tear of my Subscapularis tendon. During the waiting for Ins Co to approve my surgery I developed adhesive capsulitis. My Surgeon sent me for Physical Therapy for the last 3-4 maths to regain motion back so he can repair my tendon. Unfortunately, 8 days ago my surgeon found lots of scar tissue and didn't repair my tendon. He found little tears on the tendon( showed me pics). I work a rather physical job daily and I'm concerned if I am doing the right thing by not having it repaired. My surgeon told me mths ago after having this tear it would aggravate my shoulder and was told work restrictions no use of right hand. Should I be concerned about this tendon even though its a small tear? Should I be concerned about returning to work since I am always using my arms carrying things? My surgeon told me I can return to work in 8 wks.

Hello,

I have studied your case.

For minor tear in subscapularis tendon at age of 42 in non athlete there may not be need of surgery. Exercises and physiotherapy with manual therapy will help.

You may need to continue physiotherapy for rehabilitation
He may start TENS ,or ultrasound which is helpful in your case.
Medication like methylcobalamine with muscle relaxant and analgesic will reduce pain; you can take them consulting your treating doctor.
You may need to continue daily exercises at home also.

Some exercises which can be done after pain has reduced include- gradual neck rotation, touching your ears to shoulder, shoulder shrugging exercises, shoulder rotation, shoulder abduction.
Also check your vit B12 and vit D3 in your blood.
You can return to work now but avoid lifting heavy weights.

If no relief then you may need manipulation under anaesthesia or arthroscopic adhesion release.

I had surgery to remove the scar tissue but before this happened I had weakness in the front of my arm along with pain in the front from lifting and lowering it.

You can gain strength back by physiotherapy and exercises against resistance.
Continue your physiotherapy.
Supplements like vit C and vit B12 will help in tissue healing.

If there is no improvement by exercise and physio then you may need further investigation like EMG-NCV[nerve conduction] and MRI shoulder again.
