Suggestions on temper tantrums and thumb sucking
Detailed Answer:
Hi...by what quote I feel that he is having temper tantrums and also needs suggestions on thumb sucking. Using Femite is fine. No problem.
Coming to temper tantrums - Tantrums may happen when kids are tired, hungry, or uncomfortable; or because they can't get something (for example, an object or a parent) to do what they want. Learning to deal with frustration is a skill that children gain over time.
Tantrums are common during the second year of life, a time when language skills are starting to develop. Because toddlers can't yet say what they want, feel, or need, a frustrating experience may cause a tantrum. As language skills improve, tantrums tend to decrease.
This is what you can do to avoid or prevent tantrums -
1. Give plenty of positive attention
2. Try to give toddlers some control over little things
3. Keep off-limits objects out of sight and out of reach
4. Distract your child
5. Help kids learn new skills and succeed
6. Consider the request carefully when your child wants something
7. Know your child's limits
Coming to Thumb sucking - usually kids do this thumb sucking as a relief seeking behavior or self comfort. This is not a major problem and need not be taken into too much account till 6 years of age. Most children can safely suck their thumb – without damaging the alignment of their teeth or jaws – until their permanent teeth begin to appear that is 6 years of age.
Suggestions -
1. Try wrapping the whole palm till wrist if he does it - so that he will not be able to suck the thumb.
2. If he just keeps the thumb inside and doesn't suck vigorously - nothing will happen to the teeth alignment - so do not worry.
3. Punishing him to get his thumb out of his mouth won't help because he probably doesn't even realize he's doing it. Pressuring him to stop may intensify his desire to do it even more.
4. Children usually give up thumb-sucking when they find other ways to calm and comfort themselves.
5. If you can identify the times and places when your preschooler is most likely to suck his thumb – while watching television, for instance – consider distracting him with a substitute activity, such as a rubber ball to squeeze or finger puppets to play with.
6. Try to praise the kid if he stops thumb sucking at your behest.

Please answer my questions before sugesting on appetite
Detailed Answer:
Hi...Please answer my questions before I suggest you on appetite.
Questions:
1. What was the birth weight and what is the current weight and height (PLEASE MENTION EXACT DATE OF BIRTH TOO)?
2. How is the attaining of developmental milestones?
3. Is the stool oily and floats in the pan?
4. Is there any colour change in the hair or skin?
5. Is there any history of recurrent serious infections?
Regards - Dr. Sumanth

Weight - 3 kgs at time of birth - and now- between 11.8 to 12 kgs.
Height - 51 cms and now it may be around 83-84 cms
He got his teeth in 9th month onwards, and also started walking in 14th month only. rest all is going fine. he has a good vocab - he identifies all animals , fruits and various pictures. Speaks single words and maybe around 60-80 words he can speak. he understands all instructions given to him. he has also started making 2 word sentences like "Mumma come" or "No Mumma"
His stools are sometimes hard, and a few days back he had loose motions for around 4 days - this happened around 10 days back. But now they are normal.
No color change in hair or skin.
As I mentioned earlier - he catches Viral or any other infections easily like throat infections, cold , cough . In a month he will remain ill for atleast 15 days with the mentioned illnesses. This has been like this since last 10 - 12 months., during which we have also given many antibiotics. But even in illness he used to play and be active. Doctors whom we showed , said its because of pollution, and as he goes to school, from other children he catches it faster. His immune system is not good apparently, and as doctors have said that it will remain like this till age of 5.
No other serious infections he had.
This is related to allergy and not an immune problem
Detailed Answer:
Hi...His height and weight are appropriate. Nothing to worry about it. But by what you quote I feel that this is allergy related disease not an immune system related disease. If his immune system was not well then she should not be having this age appropriate height and weight.
All these you are mentioning are not infections and in fact I feel that they are recurrent wheezing episodes.
If you don't mind I have a few questions in this regard, before I suggest you on appetite.
Questions:
1. How many days per month does he cough or feel breathless?
2. How many nights per month does his sleep get disturbed due to above symptoms?
3. Does he feel breathless when he runs around or plays with other kids?
4. Are the symptoms when there are seasonal changes?
5. Is there any family history of asthma or any other sort of allergies like skin allergy etc.?
6. Is the cough always associated with fever?
Please get back with answers so that I can guide you better - as you have given a new information regarding his recurrent cough.
Coming to the issue of appetite.
I take this opportunity to tell you certain scientific facts and relieve your distress -
1. An active is a well kid even if she/ he is not eating well.
2. Development of a kid is as important as or I would say even more important than growth alone.
3. As the kids grow their interest in food decreases and interest in play activities increases so that they eat small quantity and run away to play. As parents we need to be more patient and feed than less quantity but more aliquots per day.
4. This age rather than the quantity of the food I suggest you concentrate more on the number of times you feed the kid and also make whatever you feed the kid calorie dense by adding good amount of ghee to it.
5. I suggest you not to use appetite stimulants on long run as they may cause drowsiness.
Regards - Dr. Sumanth

Thanks , it gives me a little relief to understand that he is growing well. Following are answers to your questions.
1. He does not feel breathless . Though coughing remains for 10-15 days, once in 2 months.
2.he generally has a sound sleep. and wakes up in morning only. Cold or Cough does not disturb his sleep generally.
3.No.
4. yes
5. Yes, his aunt (my husband's sister had asthma in her childhood but does not have now, and my husband's father had Sinus issue earlier)
6. no. Fever is only sometimes , and if he has fever , it will go in 1 or 2 days by giving Calpol.
I would also want to inform one thing that we generally travel a lot. We go to our in-laws place - around 4 hours from XXXXXXX - alternate weekends where water , weather and environment changes. Sometimes we leave him there for 2 weeks like once in every 3 months he stays there for couple of weeks, and further- weekend travelling is also there. There his eating and sleeping schedule changes, but generally he copes with all these changes easily. But seems like Dust allergy is a possibility. as it is dusty - at my in-laws town and also , in XXXXXXX - all the pollution.
Thnak you, and I appreciate you giving such detailed answers.
Looks like multi triggered wheeze
Detailed Answer:
Hi...by what you quote I feel he's having multi triggered wheeze.
Suggestions:
1. There are certainly alternative management therapies in allopathy now-a-days. Medicine has advanced a lot and allergies are 100% controllable.
2. Inhalers are the newest management strategies for this. If I were your paediatrician I would have suggested the use of Budecort metered dose inhaler (100mcg) 2 puffs twice a day through a spacer and this is for regular use for 8 weeks. Another metered dose inhaler is Levolin and this can used as rescue therapy whenever the kid is having severe cough in spite of regular usage of Budecort. The technique is very important and very crucial for the drug to be delivered correctly to the lungs. Regularity of medicine usage also matters a lot. So do not discontinue abruptly after you notice some improvement. The technique of administering an inhaler using a spacer has to be taught to you by your doctor and these are prescription medicines. So I suggest you consult your paediatrician for this.
3. Triggers can be environmental changes/ dust/ talcum powder/ seasonal changes/ un-cleaned a/c vents/ cold weather etc....we can specifically say this is the cause - unless we observe the kid closely - best person is the parent.
