The possible causes of such lumps under jawline on both sides, which go away after Zpack, are reoccurring throbbing and tender are:
-Lymph nodes
-Submandibular salivary glands.
I would suggest you the following:
-High resolution Ultrasonography
-MRI
-Specialists like an ENT Surgeons' opinion, examination.
This will confirm the diagnosis.
If lymph nodes are confirmed, to undergo FNAC.
The diagnosis is the most important part as this alone can give the proper guideline of further management.

-Since when have you got this problem ?
-Is there any problem in the oral cavity like caries tooth or loose dentures or so ?
-Do they grow in size on chewing food ?
