Question: Friend is 67 taking medication and has a pacemaker and sarcoid. A cyclist, he has had back fusion surgery and started PT a month ago after six months of rest and a few months of only walking. His able to ride his trainer for up to 30 min a day and has weekly PT. He has been feeling very tired, lethargic, a bit of an upset stomach, and a low-grade headache for a couple of days. I'm trying to get him to call his doctor.

It is a good thing that your friend is in physical activity. But these signs may be heralding signs of a pacemaker disfunction. Therefore, I would recommend you for him to check the pacemaker battery and/or the leads if they are properly placed, since pacemaker complications are rare, but not unknown. He should contact his cardiologist to get his pacemaker checked, to evaluate is it properly working, or are there any types of pacemaker-placement-related complications.

As for the sarcoid, I would recommend a plain chest X-ray to evaluate the level of damage of sarcoid caused to the lungs, at this moment. As you may know, it is a granulamtous systemic disease, which may affect all organs, but it mostly affects the lungs. However, from the description of symptoms you described about your friend, I think that the pacemeker should be checked as soon as possible, since these symptoms fit with a pacemaker dysfunction syndrome.
