Question : My 4 year old states she has an itchy tongue and she has red spots on the sides of her tongue from itching it

I have gone through your query and understand your concerns. I have checked the attached images. Try giving some B complex syrup (Vitamin supplements) twice a day for 5-10 days. Also give plenty of yogurt as it contains plenty of beneficial bacteria which should help. Also give her cetirizine syrup (antiallergic) once daily for 3-4 days.
