Suggest Treatment For Topical Dermatitis In A Diabetic
Question: I believe I may have been wrongly diagnosed with type 2 diabetes. It was in late November. I was in hospital with a skin rash, because 3 doctors (I saw previously) incorrectly/couldn't diagnose. At hospital, the emergency physician couldn't diagnose my skin rash either. She recommended a leading dermatologist, he diagnosed me with a topical dermatitis. He prescribed a cream and the rash has healed. However, the emergency doctor claimed my a1c was high, and asked if I was diabetic. I said no. She said I should see my family doctor & be tested for diabetes. Not to worry though, as I was under a lot of stress from a skin disease and prescribed cortzol. Later the same day, I went to see my family doctor, who took my blood. He later diagnosed me as type 2 diabetic with an a1c of 6.7. How is diabetes initially diagnosed? Was I wrongfully diagnosed, as the 2 a1c tests where taken the same day? Should the testes be a week apart, 3 months apart? How is diabetes initially diagnosed?

The diagnosis of Diabetes melitus is based on measurement of A1C level, fasting or random blood glucose level, or oral glucose tolerance testing.
An A1C level of greater than 6.5 percent on two separate occasions is considered diagnostic of diabetes. So,your HbA1C level is suggestive for Diabetes mellitus.
Diabetes symptoms vary depending on how much your blood sugar is elevated. Some people, especially those with prediabetes or type 2 diabetes, may not experience symptoms initially. The type 2 diabetes is diagnosed at patients more than 40 years old.
So, the diabetes can be initially diagnosed doing the fasting or random blood test and HbA1C test . Along with this, if the person has any additional risk factors, such as obesity, age more than 45 years old, high blood pressure, a sedentary lifestyle, a history of diabetes in pregnancy, high cholesterol levels, a history of heart disease, and having a close relative with diabetes.
The A1C test is a blood test that provides information about a person's average levels of blood glucose, also called blood sugar, over the past 3 months.The tests should be done 3 months apart in a person with diabetes to see the diabetes control for this period of time.
In your case, if you have two A1C measurements and both are high, in my opinion, it is not necessary to do another HbA1C test.
However, if you want to confirm if the diagnose is right or wrong , i'd suggest you to do the fasting or random blood glucose test. It is necessary to have two suggestive levels taken in two separates days for the confirmation of the diagnose.

Hello Dr. Leora Xhardo. Thank you very kindly for your response. I will definitely take your advice and have my glucose tested again, and inform you of the results. If diabetes melitus is the correct diagnosis, how should I carry on with my life? I haven't accepted any medication (as I wasn't certain I have diabetes) and would prefer to control without meds. I started a low carb, no sugar diet. I've lost 60lbs, (from 300lbs - 240lbs in 10 weeks), and started testing my blood. I find my fasting glucose higher in the morning (5.0 - 5.7) & (4.0 - 5.6) 2hrs after meals, is this normal? Is type 2 diabetes/pre-diabetes reversible? It has put a burden on me & I don't want it to affect my family. I'm a father, to 2 beautiful daughters & have a loving wife. I'm also the soul breadwinner, and want to be as healthy as I can, so I can help my family as long as a live. Please help guide me. Respectfully yours, Mr. XXXXX

But i always advice the recently diagnosed with diabetes that learning about their diabetes is the first step toward feeling better and living a longer, healthier life.
The good news is that pre-diabetes and recently diagnosed Type 2 diabetes is absolutely reversible with lifestyle changes like a healthy diet more physical activities.You are overweight and you need to lose more weight,so i suggets to continue with your hypocaloric low carb ,no sugar diet.It is good think for your metabolism at this moment.
If you were my patient, apart diet and exercises i will give you a prescription with metformine 500 mg 2x1 tb.This drug can help you to lose weight and to normalise your blood glucose fluctuations.
