Brief Answer:
You will need Pulmonary Function Testing

Detailed Answer:
Hello there, thanks for your question on HCM.

In my opinion you should first consult a Pulmonologist and get a PFT ( Pulmonary Function Test ) done.

You may have Obstructive airway disease in the form of small airway disease. Chances of this are higher since:

1. You have a long term history of Smoking.
2. Your age is more than 40 years.

So the possibility of small airway obstruction is higher. And this gives you same feeling which you are describing like limiting breathing.

A small airway disease can only be picked up by PFT.
FEF (Forced Expiratory Flow) 25-75% indicates a small airway function.
So better to consult pulmonologist and get done PFT to rule this out.
Hope I have solved your query. I will be happy to help you further.
Wish you good health. Thanks.
