Question : Unilateral swelling of right posterior surface of hand with some pain present for one month. Increases and decreases in size with no known trigger. No lymphadenopathy. No history of IV drug abuse. 52 y/o ophthalmologist. Possible trauma from bike fall on month ago. H/o HTN

I have gone through your query and understand your concern. This may be related to trauma to your right hand due to fall from bike. Elevation of hand may be helpful reducing pain and swelling. Active finger movements are helpful in reducing pain and swelling. Analgesic such as ibuprofen is helpful in pain relief. You may get x ray of your hand to see any fracture .
