Brief Answer:
few possible causes

Detailed Answer:
Hi and welcome to HCM.
First there are few possible causes. He could have had some unnoticed hit, bite or prick which could have caused hematoma. This may be allergic reaction to soap, clothes or food. If all this is rule dout then he may have some coagulation disorder so he should see hematologist. AT this point he should apply heparin gel and follow the situation. Wish you good health. Regards
