Suggest Treatment For Urinary Incontinence In A Child

Question: Hi Doctor,@
I have a twelve years old daughter that is still incontinent in the night. She also stains her undies dark yellow and nothing can remove it.
She is otherwise healthy and is seen by the same pediatrician every year since we adopted her from China when she was 15 months. Dr. Phan, ruled out any pathologic cause for the incontinence and keep saying that it will eventually disapear. I am mostly concerned about those urine stains. Should we have her urine tested?

Hi

Incontinence at the age of 12 needs complete evaluation.
I hope all possibilities would have been ruled out already.
If not discuss with your doctor regarding urine analysis, culture, urodynamic studies, thyroid levels and so on.

But if no reason has.been found then you need to concentrate on motivational therapy and pharmacotherapy if needed.
The stains should also prompt to investigate for liver and kidney function tests.

Motivational therapy includes reassurance, emotional support, eliminating guilt, and encouraging the child to take responsibility for the enuresis (i.e., although the child did not cause the condition, he or she has a role in treating it).
Simple behavioral interventions include awakening the child to void at times usually associated with bed-wetting; positive reinforcement for desired behavior (e.g., star or sticker charts for rewarding periods of continence); bladder training; and minimizing fluid and caffeine intake before bedtime.

Try simple alarm clock to awaken your child two to three hours after her sleep.

These methods if not successful may need additional therapy with drugs like tricyclic antidepressants or desmopressin nasal spray.

Discuss about these options with your doctor.
