Question: hello - I AM A 62 YEAR OLD W/F WHO FOR THE LAST 6 SIX YEARS HAS BEEN TRYING TO GET OVER A THIGH THAT WAS INJURED, BECAME INFECTED WITH A ULCER THAT FORMED UNDER THE SKIN OF MY LEFT THIGH - STAGE 4 WITH GANGRENE AND HAD TO HAVE EMERGENCY SURGERY TO CLEAN IT OUT. NATURALLY, AFTER THE SURGERY, THEY HAD TO INSERT A FOLEY CATHETER. WE FOUND OUT THAT MY BLADDER RECOGNIZED IT AS A FOREIGN OBJECT AND EVERY TIME ANOTHER ONE WAS INSERTED, SOMETIMES IT WOULDN'T STAY IN FOR 24 HOURS BEFORE REJECTING IY. NATURALLY, EACH TIME IT DID, MY URETHER WAS STRETCHED MORE AND MORE UNTIL THE BLADDER CAN NO LONGER HOLD ONE. AT THIS TIME, I AM TOTALLY INCONTINENT.I CAN'T GO FOR THE REST OF MY LIFE TRICKLING A STREAM OF URINE ALL OVER THE PLACE. THERE ARE STILL TWO WOUNDS ON THE INSIDE MY THIGH WHICH STILL HAVE NOT HEALED. IS THERE ANY TYPE OF SURGERY THAT CAN HELP ME? BY THE WAY, I AM RATHER OBESE. THANKS

To put a Suprapubic Catheter is one of the option .

Thanks for query .

Based on the problems that you have described you have incontinence of urine due to involuntary contractions of the bladder secondary to foreign body reaction (Foley catheter).

In such a situation I would suggest you to put a self retaining Silastic Foley catheter through Supra Pubic Route (SPC) which will drain your bladder continuously and prevent leakage of urine from bladder from urethra.

This SPC can be kept till all your issues like healing of the wounds and ulcer in thigh get cured .This is simple surgery which can be caried out at bed side under local anesthesia.
