Suggest Treatment For Uveitis

Question: My mother 79 yrs old and had a Phaco Surgery on 23rd Jan’14 and surgery was ok. After a week she got Uveitis as per doctor and prescribed the eye drop like Prednisolone, Vigamox. Topin, Natamet. After several months using these eye drops it didn’t cured but it gets worst and unbearable pain. In the month of July doctor asked for a Vetrictomy surgery. So I went to another doctor for 2nd opinion and he also asked the same and then on 28th July she had Vitrectomy surgery. Surgery was VITRECTOMY + IOL REMOVAL + FLUID GAS EXCHANGE + SILICONE OIL INJECTION + INTRAVITREAL (AMPHO + VORI). After Vitrectomy surgery Eye drop is using Predmet, Topin, Vigamox, Toba, Conflu, Dorzox, CombiGan. Now she lost her vision.
Now she is having inflammation again and there are severe pain and unbearable. Doctor prescribed an oral medicine Doctor schedule a prescription 40mg/30mg/20mg/10 Wysolone Tab each week. After taking each does pain is reduced but now when I stop this pain occurs again. It is true after taking this medication it is reliving the pain. Inflamation is still there. I just wanted to get rid of this Inflamation and pain. Eye lid are little swelling.
Her health is normal only is having high BP but she is under medication. Please help.

I reviewed your history.
It seems from the history that your mother had inflammation in the form of uvieitis probably of fungal infectious etiology following surgery for which she has taken treatment in the form of vitrectomy and intravitreal injection of amphotericin and voriconazole.

The treatment she has undergone for fungal induced inflammation is right.
Persistence of pain and lid swelling indicate presence of low grade inflammation. This requires treatment by the use of steroid drugs in the form of tab. Wysolone in reducing dosage.

She needs to continue treatment with tab. wysolone and needs to taper it slowly over a period of 2-3 months, so that the inflammation also reduces simultaneously.
Hence I advice to continue treatment as advised by your doctor and taper it over a 2-3 months.

Dear Doctor Dadapeer K,

Thank you for your quick reply. Is this inflammation will go away after using this Wysolone? She already had taken Wysolone 40/30/20/10mg for a month on each week. So you suggested continuing for another 2-3 months. What will be the dose? Is there any other eye drop or ointment she can use? Pl. advice.

Wysolone is needed in low dose of 10-20 mg.

Thank you for the follow up question.

Wysolone will decrease the inflammation. It needs to be taken in a low dose of 20 mg or 10 mg for at least 4-6 weeks to prevent recurrence of inflammation.
Along with the oral steroid drugs she needs topical steroid eyedrops and cycloplegic eye drops after consultation with your eye doctor.
The inflammation should decrease with these medications
