Suggest Treatment For Viral Fever, Cough And Blocked Nose

Question: Hi Doctor,

My 13 months baby (weight 16Kgs) was suffering from cold for last few days followed by a viral fever and now she is having cough and also having mucus and nose blocked.
I have gone to a doctor today and he confirmed that my daughters chest is clear and there is some URTI.

i am attaching the prescription given by him.

I am bit worried to see that he has given us Ascoril D which contains dextromethorphan,triprolidine(antiallergic like chorpheniramine),and Cetrizine is also an antiallergic.

Moreover i am checking that dextromethorphan is a cough suppressant and can cause rebound congestion and should not be given to an 13 months baby.

previously you have suggested cetrizine , but due to nose block and cough do you want to review the prescription and come up with your suggestion??

Note: Unfortunately I have already given her a dose of 4 ml in the afternoon ... Hope this one dose will not cause much harm or congestion...for the last 3 hr my baby is sleeping.

Can you please suggest the final list of medication to be given with the dose quantity starting from today night.

for last 3 weeks my daughter is suffering from cold followed by viral fever and then again cough and cold.
I have the below medicine at home now
1) T-mimic
2) Ambrodil
3) Ascoril D
4) Mucolite
5) Cetrizine

Do we need to get Ambrodil S (as doctor was saying her chest is clear, will that be required?).

My baby is not able to take solids and taking only soups and breast feed. Doctor is not able to check her throat as my baby didn't cooperate to check but i feel she is having some throat pain...

Also for better sleep at night can we use Otrinoz pediatric drop? is it safe? if we can use what will be the dose?

Thanks for asking on HealthcareMagic and for choosing me specifically to answer your query.

Let me mention that most often the mentioned complaints occur due to viral cause. Usually it is associated with an allergic component. This thickens the mucus or causes symptoms like watering of nose. The mucus needs to be diluted and mucolytics like ambroxol may provide relief. I would not suggest use of cough suppressants like dextromethorphan. Rather, giving steam inhalation should be helpful. Water in form of vapour reaches deep inside and helps in diluting the cough. If the chest is clear, salbulamol is not required.

I would suggest you to give Ambrodil, Cetirizine and Mucolite and to omit the rest. Nasoclear nasal drops will help in relieving the nasal blockage. You may use Otrinoz if further required. Irrespective of what you do the infection will take 5-7 days to recover. So, please do not panic.

Thanks for writing back. Sorry for not being able to respond earlier.

* Ambrodil - 5 ml twice daily for 3-4 days
* Mucolite - 6 ml twice daily for 5 days
* Cetirizine - 5 ml once daily for 3-4 days

Just distribute the doses between morning and evening rises anr it should be fine.
