Thanks for your answer. There was one thing I wanted to tell u that I habe observed that on both sides of tje face my son son has developed white spots. . carn this be a side effect of steroid? They r not rashes but white spots. I have attached a snap for yourï reference.

These white spots are a fungal infection... TINEA. Yes it can happen in patients on steroids especially during this season where there is a bit of moisture and less of heat. Lot of people get this infection. It will be probably spread all over his body.
Dont worry about it. Start him on tablet Syscan 150 mg once a day for 5 days. If it doesnt improve you can consult a dermatologist. But dont cut down on your steroid dose for it. It is a minor problem and will be taken care of.

we got another urine report (protien creatine ratio) as being another 15 days from the last. The fig is 735 as compared to 385 last time. But have a look and compare the spot figures. Since this is a ratio is it possible that the increase is a result of the reduction in creatinine again as a result of hydration? He did drink a lot of water before test.

Firstly apologizes for the late reply. Had my daughters 40 day function being organized at home. So was totally occupied.
I have gone through the results. There seems to be something wrong with it as the Urine protein and Urine creatinine concentration are at the lower side this time while the ratio is higher. I checked through the other 3 results and this time it seems to be at the lowest as compared to the last ones but the ratio is somewhere high. Sorta of a confusion. But nothing to worry about it as of now.
If the spot ones are coming down , the UPCR should come down as well. You can try repeating it after 15 days once
PS: You are under a follow up of a very experienced rheumatologist. I am sure aniruddh will be taken care of. As per the reports he is heading towards betterment only

The reason i wrote you about that report is because i myself observed that with lot of water intake before test reduces the spot createnine level and increases the ratio. May be createnine being the denominator. Thats the reason I repeated the same test the next day at about 9:30 am without much water intake an look at the result i have attached. Remember its just a matter of one day difference.

Well, yes it probably true with adequate hydration the levels will vary. That is the reason it is mostly used as a screening test than something which is used to asses the kidney function. The gold standard still remains 24 hour urine protein as it assesses the protein loss over the whole day..
Even with infection or dehydration, spot urine protein creatinine ratio can vary. I would still suggest, keep that as a screening. Whether he needs more aggressive therapy or not, always take the decision after 24 hour urine protein.
It will take quite some time for protein leakage to go down as it is the last to revert. But as long as it is not going up, it is a good sign
