Question : male 72 yrs old. very smelly bright red solid rash left side of groin covered by testicles. Isn't warm. Doesn't itch.

I read it carefully and I understand your concern.

Solid rash, bright red, smelly and in that location most probably indicates a fungal infection, which not always can be itchy.
I will recommend using Ketoconazole 2% cream, twice per day for 2 weeks.
Also, try to avoid as much as possible the sweating there.
