Question: male 72 yrs old. very smelly bright red solid rash left side of groin covered by testicles. Isn't warm. Doesn't itch.

Solid rash, bright red, smelly and in that location most probably indicates a fungal infection, which not always can be itchy.
I will recommend using Ketoconazole 2% cream, twice per day for 2 weeks.
Also, try to avoid as much as possible the sweating there.
