Suggest Treatment Options Other Than Knee Replacement For Controlling Knee Pain Due To Arthritis In A Diabetic

Question: Dear Sir,
My Mother Is having Knee pain(Arthrits) and it is becoming difficult to do day to day activities.Atached the Scaned copies and xray of Knee to thi.
Request you let me know whether it can be controlled by any medication or knee replacement is only option.
My Mother is having diabetes from past 13 years.Whether diabetes patients can undergo knee replacements. Wanted to confirm if we go for knee replacement whether it would have any pre or post surgery Complications due to diabetes.
also can you please refer to some Good Experience doctor in XXXXXXX who is having good success rate in knee replacement surgery.

I have seen all the attached reports in detail. Your mother has osteoarthritis in both the knees.

For osteoarthritis,
1. Weight reduction is the main treatment along with medicines to decrease pain like NSAID's - Paracetamol, Aceclofenac etc.,

2. Life style modifications like avoiding sitting cross legged, squatting, using Indian type toilets will decrease the rate of progression of osteoarthritis.

3. Exercises like quadriceps strengthening, swimming, straight leg raising will help in improving the muscle strength around the knee and will help in decreasing pain.

The treatment options include pain killers, hot fomentation, lubricating agents like Glucosamine sulfate, Chondroitin , Dycerin, etc.

She needs to start with joint lubricant supplements and see if there is any relief after taking proper supplements for 3-6 months.

In severe cases intra articular injections of steroids and Hyaluronic acid may be needed after consultation with your Orthopaedician.

Surgery is the last option when all these measures fail to provide relief and patient is in constant pain.

Replacement can be done with diabetes but it is best to delay the surgery with the help of medicines and physiotherapy as much as you can.

Thanks for your input and giving alternative ways to reduce the pain.When we consulted orthopaedic doctor they directly suggested to go for knee replacement since it was in last stage.If it is in last stage whether medicines suggested or injection will have any impact. My mother age is 60 Is this correct age to undergo knee replacement.I mean what would be ideal age for undergoing knee replacement and how long the implants would last long after doing knee replacement

The stage of osteoarthritis depends on the pain and symptoms.

If the pain can be controlled with pain killers then the injections and regular physiotherapy will definitely decrease the discomfort.

There is no ideal age to have the surgery for knee replacement. Ideally everyone should do a regular exercise to reduce these degenerative changes.

After a knee replacement, the implants usually last for 12-15 years. After that a revision surgery is needed.

Could you please confirm the Injections and Pain Killers available for osteoarthritis Knee pain.We will follow your advice before going for Knee replacement.

Also wanted to confirm whether osteoarthritis effects only Knee or ankle as well .
Reason im asking is that My mother is facing pain in ankles and will be able to walk only with support whenever she gets up in morning from past 2 days.And this would gradually be ok as day progresses.
Wanted to confirm if there is any Home remedy for this or what can done to avoid pain when she gets up in morning.

Hi!

Osteoarthritis affects all the weight bearing joints including hip, knee and ankle.

She needs proper medicines after careful assessment of the degree of pain and disability. This needs a clinical examination.

I can only recommend you the general medicines but dosage regulation needs regular monitoring of symptoms by an orthopedician.

She can start with Tab. Torcoxia Bcd 90 once a day or tab. Lorcam P twice a day after meals for pain.

The joint lubricant supplements that can help her are Tab Cartigen DN twice a day with Cap Mienta twice a day for at least three months. She needs to add calcium supplements like Tab Supracal OS also.

The injections will be needed if these oral medicines and a regular physiotherapy schedule does not help.

Regular hot fomentation of the painful joints is also recommended.
