Suggest Treatment Severe GERD And Gastritis

Question: Hi please help me. I have severe GERD and the last 2-3 weeks its to the point that water comes back up after drinking it . I have been on nexium for 2 months 40mg daily and I just started zantac. Nothing is working!!! I am so scared and nervous, you see I have lupus, but its very mild. . . I had an ANA = 1:160 speckeled in the past (1:160 is the highest it got) . I am worried due to my gerd that I may have scleroderma ! my scl-7o and anti-centromere labs are negative. I do not have raynauds or skin issues. I had an EGD 2 years ago that showed gastritis only. I don't know whats wrong, and its starting to make me scared , especially since the meds are not working , please help me.

Hello,

You are on an appropriate dose.

I would advise you to continue nexium for a period of 4 weeks in case your symptoms persist at the end of four weeks, I would suggest to repeat UGI endoscopy to check for erosive GERD

I would advise you to
Avoid fried fatty foods, carbonated drinks, alcohol, caffeine, mint and chocolate flavoured foods.
Avoid lying down for 3 hours after meals.
You can take a liquid antacid like gelusil on an as required basis.

Do get back in case you have further queries

is it possible that I could have scleroderma ? and that is what is causing my esophageal stuff? I have had negative scl-70 and negative anticentomere antibodies, and no skin issues and no reynauds, but because I have lupus I am very afraid of having scleroderma , and I know GERD and swallowing issues can happen with scleroderma, EGD 2 years ago showed gastritis.

Hello,
Scleroderma is unlikely.
In case your symptoms do not respond we need to find the cause of your symptoms whether it is due to GERD or dysmotility.( ineffective movement of the esophagus)
He next step would be to get a upper GI endoscopy done if normal the next step would be to get an esophageal manometry and pH studies.
