Posted on Mon, 7 Apr 2014
Question: Hi, My daughter is born on 7th Nov 2013. It was an elective LSCS and she was born on 39 weeks. She was born 2.84 kg and everything is normal. Till 1 month she kept on reducing weight to 2.3 kgs. Then our doctor asked us to top up with formula feed along with bf. So since she was 1 month she is bf and also formula feed. After 4 months also she weighs just 5.1 kg which is underweight for her age. We have tried different formulas available in market but she doesn't like formula. Bf is not sufficient so we are giving her forcibly. She is also given the below medicines I have mentioned. Apart from weight everything else is fine. Can you please suggest something to increase her weight? Is there something to worry?

Brief Answer: There is no cause for concern Detailed Answer: Hi Welcome to Healthcare-Magic Greetings of the day Namasthe Dear XXXX Being a Parent myself ,I understan your concern and apprehensions. Let me first reassure you, there is absolutely no cause for concern even in the aspect of weight. Ideally the babies double their birth weight at end of 5 months, that is at 21 weeks of age. Her birth weight was 2.84kg, so it sould be 2.84*2= 5.6Kg at end of 5 months(21 weeks of age). She is 17 weeks presently 17 weeks of age and is weighing 5.1 kg, so she should definitely put on 0.5 kg in next 4 weeks time. Do follow the advise given by you Pediatrician. Also do remember management of children is not all about values and weights, as there is wide variation in growth among the children. Adequacy of feeds is assessed by many things and weight is just one of the criteria 1. Baby sleeps for atleast 2 hours after feeds: It implies the baby has adequate satiety 2.Passes Urine 5-6 times a day 3. Other milestones like neck holding has been attained. I would also suggest you the following to increase you milk output 1. Capsule Lacteer thrice a day will be helpful 2. Shatavarex powder: 2 spoons in glass of milk thrice a day 3. In a pan add 2 spoons of Desi Ghee, once its hot, add 2 spoons of whole wheat. Make it golden brown. Add 3 cups of water and boil for 2 minutes at low flame, avoid complete drying. To this add finely grinded mixture of Pista, Cardamom, Khajur and almonds. Mix it well. Khajur and cardomom are known to increase milk output. You can eat it with your breakfast and at night after dinner. 4. Reduce anxiety and stress. 5. Start weaning diet after 5 months of age In the absence of any other symptoms, there seems to be no cause for concern. Do write to me directly if you need any further assistance, will be glad to assist you. Take Care Best Regards Dr Deepak Kishore MBBS,MS,MCH Consultant Pediatric Surgeon

Hi Mr. XXXXXXX XXXXXXX Thanks a lot for your answer. I will try to do as per your advise. Just a small doubt. Actually she will be 19 weeks tomorrow and she was 5.1 kg 1 week back. As per her weight gain pattern I don't think she will be 5.6 kg in 2 more weeks. The medicines I had mentioned is it all mandatory to give? Our doctor has told to stop Calcimax and to continue the rest of the medicines. As you have mentioned above her sleep, passing urine and neck holding everything is fine. Can you please suggest which formula will be good though we have tried almost all of them. And also how many times and for what duration should I feed her and how many times formula I should give? You have also mentioned to start weaning diet after 5 months. Usually its after 6 months I have heard. So is it fine to start at 5 months and what should I give her. Can you please give a schedule as per which I give her solid and milk and what and how to prepare her food? I am a first time mother so may be I have a lot of queries sorry for that. Thanks & Regards, XXXX

Brief Answer: Simly MCT will be helpful Detailed Answer: Hi Dear XXXX It is not mandatory that she has to be exactly 5.6 kg, if she is somewhere close its acceptable. Exclusive breast Feeding is to be done till 6 months of age, as you have already started her on top feeds, its much more necessary to pre-pone weaning by 1 month as it will induce catch up growth. Continue with the advice of your Pediatrician, also I would advice Simyl MCT drops 10 drops 4 times a day. It contains medium chain triglycerides which are rapidly absorbed and help in weight gain. I would suggest you home made weaning diet based on your cultural background and beliefs. Let the feeing be on demand, feed whenever the baby is hungry. I would like to share few of recipes which may be helful to you. You can experiment and give which the baby likes. Initially start with cereals and milk, latter fruits and eggs can be introduced 1.Veg soup: Contents: tomato -2, potato-1small, Onion - 1 small, Carrot-1 small. Preperation: Wash all veggies properly peel potato, onion and carrot. Boil tomato in water for 5 to 6 mins sothat u can see the skin of tomato cracks. Now gently remove skin of tomato. Add tomato and the peeled veggies to a cooker with 1/2 a glass of water. Add pinch of salt and whistle the cooker for 2 to 3 times (veggies shud be cooked well). Now bring it to room temp and blend them in a mixer. Strain the contents to remove any lumps and now boil the soup with cinammon sticks (1or 2piece very small one) added to it. Boil it till it reaches the required consistency and then feed LO. Note:You can add any veggie to make this soup including leafy veggies like spinach, amaranthus, cabbage etc. 2.Pumpkin: Take peeled pumpkin. dice the pumpkin and then presure cook it with little salt. mash well with hands and feed it along with rice or as such. 3.Gourds (any ashgourd, rich gourd etc etc): Make it finger size and then pressure cook it or boil in water till it becomes soft. You can either feed as a finger food or you can mash it and feed it along with rice or as such. 4. Boiled potatos and carrots. Pressure cook well and mash it with hands and add pinch of salt and feed baby as such. 5. Caramalised plums: Peel the plum. Put it on hot tawa and get it caramalised on both sides and then feed it mashed. XXXXXXX also u acn do the same way. 6. Poached pear: peel and pit the pear. Take 1/2 glass of water and add cinammon sticks, grated garlic, cloves etc and bring it to boil. Slip the pears into this boiling fluid and cook until its soft and the fluid is just about to disappear. Now remove all the sticks garlic cloves etc from the fluid and mash the pear in the same fluid and can feed the lo. Its really yumm. And for elders you can add sugar and honey while boiling which makes it a decent desert. 7.Avocado XXXXXXX puree. Take avocado and peeled and diced XXXXXXX in equal qty and grind well in blender. Directly feed the baby. 8.Banana: Ripened banana made to thin long slices. Heat tawa add little butter or olive oil and then saute the banana until its soft. mash and feed. If you wish you can add bit of sugar or honey to this 9.Oats and XXXXXXX breakfast: ground oats and diec XXXXXXX Boil apples until its soft and add ground oats cook until oats is done and a yumm breakfast is raedy. 10.Daal : Cook daal in cooker with salt and curry leaves added to it. Mash it add few drops of ghee and u can feed it either with rice or as such You can still write directly to me anytime even after closing the discussion, will be very glad to be there for you. I do understand difficulties of first time mother. Wishing you a happy motherhood. God bless you and your family. Take Care Best Regards Deepak
