Suggest Treatment To Remove Uterine Fibroid

Question: Dear Ms. XXXXXXX

I don't know if you can see my previous questions. I had yesterday a hysteroscopy as the doctor told me that on the ultrasound it showed that I had polyps. It turned out that those polyps were actually a big fibroid (it fills all my uterus basically). I will take the photos with me tomorrow at work and scan them and send them to you. Doctor sent me for an MRI scan and I will see him again in 2 weeks time, on a Friday to discuss what next.

I am now more worried- will this mean that due to the size of the fibroid (if it is indeed fibroid and not a tumour) I will have to have a hysterectomy?

I am worried because I am scared of general anaesthesia as I think I might suffer with sleep apnoea. Also my blood pressure was sky high. They gave me sedation yesterday and I was aware of everything what was going on and my blood pressure when I came out of the operation room was 190/102. I do suffer with high blood pressure and I will probably need to increase the dosage.

1. Please let me know what procedure could be available for me to remove this huge fibroid?

2. Is hysterectomy dangerous? Can people die from it? What are the side effects?

3. Can I try hormonal treatment and see if the fibroid shrinks?

PS- how soon do you think I should have this removed? Can I wait until I find out if I suffer from sleep apnoea?

I have gone through your history and these are my opinion about your query.

The fibroid is a benign growth in uterus and its treatment depends upon symptoms, age and fertility.

-Hysterectomy is not the only treatment of fibroid, but yes, it is a definitive treatment at your age.
-You can go for a nonsurgical method for fibroid treatment after consulting your treating doctor-uterine fibroid embolization (supply to fibroid cut off so it shrinks) and GnRH analogue injection for fibroid.
-No method is 100% safe, but hysterectomy is now a day a safe surgery in expert hand and usually done under general anesthesia.
-Complication after hysterectomy are very less but usually bleeding, infection, pain and so forth
-No there is no need of immediate hysterectomy, you can first confirm whether you are having sleep apnoea syndrome or non.
-Maybe hysteroscopy was done under local anesthesia, thus you were aware of everything.
-First do not take stress and left your medical illness with your physicians and let them decide what is the best option for you.
-I can see your concern about surgery because you are suffering from so many medical illnesses and taking treatment, but please leave everything upon your treating doctor and take your medication in appropriate doses to maintain your blood pressure, asthma, anxiety under control.
-Maintain your weight with a healthy diet and work out regime.

Many thanks for your quick reply. I just got a call from my doctor. I had asked him if he took samples from this fibroid to send to the lab and see if is cancer. He told me that he did not take samples as my fibroid is too big and this is why he sent me to MRI to see if is cancer. I am very very worried. Do you think it might be cancer? My blood test results are all perfect and I do not bleed between periods. If it is cancer, what are the chances of survival from uterine cancer?

May be it is not uterine cancer.

Hello dear,

Nice to hear from you again though.

The probability of uterine cancer is less in your case as you are not experiencing any symptoms related to cancer and your rest of the investigation are fine but nothing is 100%.

So please hope for the best and if you diagnosed with uterine cancer, prognosis is good because 5 yr survival rate with uterine cancer without spread is 95%.

If at the time of diagnosis, it spreads locally 5 year survival rate is 68% and if spread to distant area survival rate is 17%.

You mention 5 years survival rate. It means there are no records as to what happens after 5 years or the maximum survival rate is 5 years (in each of the circumstances mentioned)

Statically for cancer, we use 5 year survival rate. It stands for the percentage of patients survived over 5 years. It does not mean that the patient only survives for 5 years, sometimes a patient survives for 5 years and cancer never come back and in some cases patient died before 5 year.

Sometimes 1, 2or 10 year survival rate also be practiced in different cancer.

Your hysteroscopy shows a huge fibroid, it needs surgical intervention.
