Suggest Treatment When Two IUI Treatments Were Unsuccessful?

Question: Hello Doctor,
I have undergone IUI treatment two times in November & December 2014. Both are failed. (My age is 28 and weight is 71Kg)

In November, Doctor prescribed me Good Ova plus-from day 2 (25mg). When my follicle size reached 23mm (right side) + endometrium 11mm doctor gave me HCG trigger one day before IUI.
Same treatment repeated in month of December, 2014. Both IUI was performed on 16-17day of my cycle.

In January 2015, doctor prescribed me Good Ova plus (50mg) to increase follicle size fast. But in this month, there are two follicles 13mm and 14 mm on left side on day 13. But on 16day my follicle stops growing. So my doctor cancels IUI for this month and asks me to take tab Deviry (10mg) for next 7 days.

Here is my past history: I had hemorrhagic cyst rupture in Sep, 2013 and has PCOD problem in Oct 2012. My menses are regular with XXXXXXX 3-7 days delay. Other than these two issues in past, now I don’t have any other health issue (also don’t have irregular periods problem). My HCG reports are also normal too.

My husband has OAT problem (Grade II varicocele). As per doctor at the time of both IUI his reports are normal. Doctor told me that we both have normal reports and I’ll conceive in 3-4 IUI.

I’m worried and confused now..! Need answers of below question.
Even if my doc increased my tablet dose why follicle stops growing..?
Is there any specific reason for follicle issue (stress, medicines, etc.)..?
What are my chances to conceive with all these reports?

Hello

I understand your concerns about trying for pregnancy.

Let me tell you that chances of pregnancy in each cycle of IUI is only around 15 percent.

Non development of follicles could be due to insufficient dose of gonadotrophins .

I suggest you request your doctor for addition of gonadotrophins to improve size of follicles next cycle.

Please attach scanned copies of your husband's semen analysis report during IUI and before IUI.

Please find attached semen analysis reports of my husband.
I’ve mentioned date on each report. First report (26/08/2014) is after taking 3 months medication- fertomid (25mg), conceva M, and one more tablet.
Doctor asked him to continue the same medicines till I conceived.
Is this a major problem..? Any recommendation..?

Hello

Stress is likely to be a reason for not improvement of follicle size. Weight could also be a concern . Since 71 kg weight is on higher side it is possible that overweight might have prevented from increasing follicle size.

There is also possibility that there was wrong measurement of follicle size either last time or now. If follicles were growing right size with 25 mg there was no reason for increasing the dosage.

This is not a major problem. This is seen once in a while. All you need is addition of gonadotrophin injections as discussed before.

Your husband's semen analysis is good for conceiving. Weight loss should greatly help you in conceiving.

Do you recommend any tips for the same..?

One more question - only one mature follicle is enough for IUI OR 2-3 are better..?
‘Left ovary sacculation’ is mentioned in my HSG report... My doctor says, it’s normal.
As per my observation, I always have mature follicle in right ovary and if I have it in left ovary it stops growing after 13-14mm. Is it because of ‘Lt Ovary sacculation’..? What does it mean..? Does it require separate medication..?

Hi,

Please try to have low calorie diet of about 1000 calories a day and exercise twicw a day for 30 minutes.

Having 2 to 3 follicles is better than one. There is nothing called left ovary sacculation which might lead to non growth of follicles. If follicle is not growing you just need gonadotrophin injections for growth of follicles.
