Brief Answer:
Timing dosage of medicine should or autonomic dysfunction

Detailed Answer:
Two reasons generally explain such situation one could be timing or dosing may not be appropriate and need to changed and some time even medicines itself need to be changed due to peak and through effect. Other explanation could be effect on nerves what we call autonomic dysfunction. As she is symptomatic need immediate attention. Have a ambulatory blood pressure monitoring done. It will give exact idea as to how pressure is fluctuating and medicines can be changed accordingly. If there are pointers to autonomic dysfunction there will be need for further evaluation and treatment.
