Question: Question for Dr XXXXXXX Varshney. I recently asked a further question which went to another doctor with no continuity of advice. Having followed your advice regarding the control of my BP i still find that the old problem of getting hot flush like symptoms in the afternoon and evenings persist. I consulted a GP and explained that i do not feel too well with low energy, low libido and erectile disfunction. The GP has got me an appointment with a uroligist as my prostate has been watch and wait for the past ten years. It hasnt changed but i have wondered about testosterone levels and will insist he checks. I wondered what your take on this would be as your advice has been helpful in the past.

Detailed Answer:
Welcome to Healthcaremagic.
Hi, I am Dr Anshul Varshney , I am glad to reconnect with your.

See, at this age low libido is a natural thing. It happens with almost everyone.
Testing for testosterone levels is not at all an issue. But my point is that , what is the use!

Suppose, your testosterone levels are low in your blood test, then at this age there is no point of loading you with testosterone injections.

Remember, every medicine comes with it's side effects.

My best opinion this regard for you would be to take Sildenafil or Tadafil whenever you wish to have sex.
However, we always recommend a cardiac clearance and a controlled BP before taking Sildenafil.

Now coming to lack of energy and hot flushes. Yes, this all could be due to Andropause.
However, again, my point is that there is no indication of Testosterone replacement therapy.

These all symptoms respond very well to SSRI like Paroxetine. However, in your case i would not recommend that too, as these SSRI in long term can cause Erectile Dysfunction that you are already facing.

Get your Hemoglobin, thyroid levels, blood sugars, Vitamin B12 levels done as a routine check to rule out any other cause for low energy.

Yes, i would recommend for you Methylcobalamine supplementation as it helps people having erectile issues.

Do morning walks, yoga and meditation to get rid of hot flushes.

This is my personal opinion based on details available here. If you still have any other query, you may please ask me.

Stay Healthy.
