Attached is a little more info which answers several of your questions. I will work on the rest now.- Sinzce when problem is there? Plane crash, spinal cord damage

10/31/2011; my interest in sex left me somewhat gradually over the first year or two. Orgasms as well, although YYYY@YYYY never carried the pleasure and intensity as those from before the accident.

- Do you have problem of low libido (desire for sex) or erectile dysfunction or both?

Both, although Viagra takes perfect care of the erection dysfunction.

- Do you have any other physical problem like diabetes or blood pressure?

Ah, yes... I take 5mg of am lodi pine benzylate (spelling) for high blood pressure. Was 10 YYYY@YYYY but I reduced it to five when pressure dipped to around 60/100. Today I am 80/120 nearly every time.
No Diabetes.

- I need to check your MRI scan spine report. Please upload or narrate conclusive report.

Will try to get mri and report, but the plane was a flimsy ultralight which I lost control of. While in a death dive (straight down, with engine likely to plow into my back and kill me as I planted the nose of the craft into the ground) I miraculously flared the plane at the last second allowing the shocks to absorb some of the downward force, but not enough as my "back bone" was compressed until my T12 burst under the pressure, chards of bone cutting, bruising and lodging into my spinal cord at the T12 area. The SCI was classified as damaged incompletely, as it was not sheared in two.

- Do you have habit of smoking or alcohol?

No smoking, love my light beer for supplemental pain relief and bladder flushing. I have frequent uti's but seem to do better when drinking lite beer in moderation. Never do I drink in association with sex. Weekly total would be about a case of 30 coors light per week.

- Is there spontaneous morning erection when you wake up from bed?

Rarely, but sometimes... one or two times per month.

There you go. I was going to try Invitro fertilization, but could barely produce enough orgasmic fluid to to measure a sperm count. Results were <3% viable sperm in a tea, yes teaspoon of fluid it to me 90 minutes to produce. And these 3% had slightly damaged tails and heads.

Fyi: When I do have an orgasm by master baking < one time per month at best., it sometimes unpleasant with a warm wet sensation in my Anusara and rectal splinter. Not much to celebrate.

I will be happy to help after your response.

Dr XXXXXXX xxxxxxx
In last paragraph that starts... Fyi

Should read '...have an orgasm by "masterbating" < one time...'

and later '...in my "anus"...'

< of course represents "less than" in this discussion.

Brief Answer:
it is really a complicated case, supplement may help

Detailed Answer:
Hello,

Thanks for information. Sorry for delayed response.

I can understand your situation of suffering. Still it is really good to hear that you are getting erection with viagra.

Your case is somewhat complicated as there are many factors working. Opioids are highly related to low libido after long use. Duloxetine in few patients can cause low libido. Low libido can be part of Depression also. And above all your injury also affects erection and in turn libido.
Low testosterone can also be responsible for low libido so you should go for fasting morning serum testosterone (free and total) level. If it comes low, we can provide supplement.

As such there is no specific treatment of low libido. Each etiology should be addressed but it is not feasible n your case.. I would like to mention one supplement for that. You can use medicine cum vitamin supplement containing pyridoxal-5-phosphate, mecobalamin and l-methylfolate (trade name tetrafol plus or metanex) is very helpful in my patients. It has shown improvement in 50-60 patients of mine with ED having hypertension & diabetes. You may get this medicine over the counter as it is vitamin product. Take it for at least 20 days twice a day after meal and if you feel some improvement continue for 2 months.

Take care.
