Question : I have type 2 diabetes, and am taking Januvia 100 mg per day, and my sugar numbers continue to rise today..last 5 minutes it was 487...What can I do to bring the number back down?

Thanks for contacting HCM with your health care concerns and questions.

You are concerned that your blood sugar keeps elevating despite talking both Januvia and apidra. The Januvia is an oral medication that usually takes awhile to get full use out of. If you have been on Januvia for more than 1 month then you are getting its full benefit. Taking more than 100mg will not help in lowering your blood sugars

The apidra on the other hand is a quick acting insulin that we usually dose with your meals. to know how to dose your insulin appropriately you should contact your diabetes doctor and get proper changes through them.

The elevation in your blood sugar could mean that you have an infection somewhere and your body is trying to fight that infection. For this again I recommend you call and talk with your doctor so they can make proper recommendations. In my practice with people on apidra and are having elevated blood sugars I get a urine test and ask if there are any stressors going on even home stress that may affect your blood sugar. If there is no additional stress or infection then increasing your dose of apidra maybe needed. I would also recommend starting a daily long acting insulin like lantus or levemir to help reduce your blood sugars.

Since diabetes is such a complicated disease before I make any recommendations on your insulin or further treatment options I recommend you see your doctor and get their opinion first.
