Suggest Ways To Wean Off Lisinopril

Question: How to go off my Lisinipril (sp?). My BP was consistently 105-108 over 65-69 while on 10mg for about a year with a LOT of work, weight loss and supplements. My research said the average drop in BP from Lisinipril was 6 on the top and I think 4 on the bottom and while my BP was up (130) at the doctor (long story) the PA agreed I could come off or drop from 10mg to 5. So when I went off, the first morning was 120/70 or so which was OK. 2nd day not so good. Not sure the real number (took 2 times on 2 BP monitors) and it was between 135 and 150 top. 70-75 on the bottom. I don't understand what happened. Thought worst case I would be 10 or so points higher on the top no.

Hello!

Welcome and thank you for asking on HCM!

I read carefully your question and would explain that your actual blood pressure values are below the normal ranges.

Nevertheless, the dose of lisinopril that you are taking is a low dose and the effects of dose lowering (such increase in blood pressure) are a little inexplicable.

But, only a blood pressure measurement is not really meaningful. A close monitoring of your blood pressure values during the day is necessary to come into conclusions. Physical activity and anxiety can cause an elevation of blood pressure.

I would recommend reducing the dose to 5 mg daily for at least 5 days.

You should know that every change in your anti-hypertensive therapy needs up to a week to show its real effect and bring your blood pressure to a stabilized new level.

I recommend to closely monitor your blood pressure frequent times during the day (usually in sitting position after 10 minutes of total relax) and refer those values to your doctor.

If after a week your blood pressure is still low, you can try to make a further reduce in the dose to 2.5 mg.

Hope to have been helpful!

Kind regards,

Dr. Iliri

To be sure I understand, you feel when going off my med (or down in Rx level) that my BP may spike initially but may settle to a lower level after 5 days or so?

Detailed Answer:
Hello again!

In fact, such a low dose of lisinopril does not explain the spike in your blood pressure.

Nevertheless, it is true that a stable blood pressure is achieved after some days (when your body reaches a new balance with the drug in a certain dose).

If your blood pressure remains high, since the first days, you can try to increase the dose with 2.5 mg, in order to have a more gradual decrease in the dose, to avoid such adverse effects.

Hope you will find this answer helpful!

Greetings!
