LDL is still high - in spite of life style changes.
You should start taking a drug - generally one from the statin class (example Rosuvastatin 20 mg / or atorvastatin... )
Repeat the tests after 6 to 8 weeks and adjust the dose.
Vitamin D deficiency is easily correctable - tablets 60,000 units once a week for 4 weeks and then daily maintained at a small dose...
The cause of anaemia needs investigation - considering the long duration, a haematologist could identify some unusual causes....Iron, B-12 deficiency, mild forms of inherited disorders and so on..
It is good to be vegetarian.... Check for B-12 after an year...
