Question: Tested positive for herpes several years ago but I’ve never had an outbreak. However I now have slight pain inside my penis, swollen lymph node in my groin and it’s like my Lower back is aching. Is this an outbreak without physical signs or something else?

I read your query and here is my advice.

The swollen lymph nodes together with back pain and pain in penis are clearly signs of infection. It can't be said if it is herpes if it doesn't have its characteristic fluid filled vesicles and rash.
It can also be any other type of genitourinary tract infection.
You better do an uroculture to check for it.
