Question: Q1. This is for my 6 month old (precisely 5 month 28 days) old son . His urine RE and culture has shown presence of E Coli bacteria. His symptoms for last few days include- irritated, touches penis when urinating and cries, little cough. Doctor has suggested Augpen HS Suspension 3 ml twice daily for 7 days and USG for abdomen. We are very much worried for E COLI infection for so small baby(weighed 7.3 kg,). Kindly let us know if this medicine is proper or not., Whether the antibiotic will cause any harm to so small kid(6 months).

Q2. My 3.5 yrs daughter often catching fever since July end this year. Almost every week for 1 or 2 day she gets the fever and after applying paracetamol it goes away. Her weight is 15 kg. As a result she is missing school very much and her normal activities getting affected. Kindly suggest which tests we should go to find out the cause of recurrent fever.

Brief Answer:
Please upload the urine report

Detailed Answer:
Hi,

Let me assure you that urine of all individuals (including infants) contains E.coli and similar bacteria. It is the count that is important. I would insist you to upload the report. Unless the colony count is greater than 10^5 (i.e. 100,000) per cu.mm, you need not worry. In case treatment is indicated, Augpen HS can be given but 5 ml twice or thrice daily would be the right dose for the child.

Regarding your daughter, I would not insist on any test since since she seems to have low immunity and testing will not help. However, you need to discuss with her treating physician whether he thinks tonsillectomy to be needed. I would however insist you to get a blood allergy profile done. Based on the inputs I would guide you further.

I have uploaded the reports. Kindly comment.

The fever in my daughter is a recent incident. In last one month she got fever thrice in a gap of one or two weeks. Otherwise earlier she was ok. How can we increase the immunity. Such fever is painful for her or for us too as she becomes sick and her normal works are hampered.

Brief Answer:
Urinary tract infection present

Detailed Answer:
Thanks for writing back. I have checked the reports. Your son has urinary tract infection by E.coli and the infection is resistant to most antibiotics. Giving Augpen HS would not be sufficient. You need to add Syrup Nitrofurantoin (25 mg/5 mL) 2ml four times a day to be given for 14 days.

Regarding your daughter, there is no way to boost her immunity apart from giving her plenty of fruits and vegetables to eat. Possibly she is getting viral infections. Please get her tonsils checked.

Regards

Sir, this time the fever in my daughter(3.5 yrs) reaches to 104.2 after giving P250 5 ML syrup. Maximum for 4 hours the fever is under control , but after that it again rises. Since yesterday evening she is suffering from it. Already 3 doses of P250 administered. Fever comes with fatigue and body aches.

We have given for urine RE and CS for tetapi today.

Brief Answer:
Ibugesic can help

Detailed Answer:
You may give Ibugesic if fever is inadequately controlled using P250.

The repeated urine test will not provide additional information. I have already mentioned the requisite treatment.
