To Understand The Root Of My Issue, I Think There

Question: To understand the Root of my issue, I think there could be a structural, or a defect with the colon. At random times Poo will get stuck in the Sigmoid colon and just build behind that, into incredible discomfort. And if I don't go to the toilet and push as hard as I can, it will just build and build. And sometimes I need to stand up, and press on my body, and to get stuff to come out. I never used to have this issue, it used to be a normal, need to go sensation, as if the "Poo" Dropped at the bottom of the rectum/colon part. And go to toilet and it would come out with ease. But over the last 6-10 months, it seems to get stuck behind that and it doesn't drop to the end part.

"8/12/2019 8:00PM Went to the Buffet, had two plates of food.
Around 9:00 I felt increasingly pain in the lower left abdomem, with a fullness sensation, and incredible discomfort.
Around 9:20, After straining very hard on the toiet, I manage to pass a tiny amount. Around 10:00, Pushing as hard as I can,
and a large clumpy chunk comes out. 10:30 - Went to washroom again, and strained a bit, and another amount came out. (Feel relieved Better By 11 PM)" "During this time, Felt I was going throw up, like my body will implode inside."

Fairly embarrassing, but I think I may know what would of caused it. I think I may of caused structural, nerve, or the muscle functionality of the colon for pushing the stool part.

See, I used to have a strange fetish desire, and I used to rub my pelvic/male pleasure organs/ Against a flat floor as hard as possible. Sometimes tucking it into into the body, and rubbing myself into the orgasm/masturbation was done. I have done this for a long time.

But like 10-15 months ago, one night I did it, I felt a very strange shake feeling in the lower left abdomen like a bad pinch, hard to describe, but as if I may of damaged the colon. (I haven't done that fetish thing for over a year now, and never will do that again).

But Ever since then, there are serious issues with the digestive system/tract. Would a CT scan find an structural/nerve/damage to the colon? As had one 6-9 months ago with nothing.

Do I need more investigation, colonoscopy, or other stuff. To check if perhaps there is fat protruding into the area pressing on the colon, or a damage?

Brief Answer:
Treatment for constipation is needed- colonoscopy may be indicated if there is no improvement

Detailed Answer:
Hello and welcome to Ask a Doctor service,

I carefully read your query and also checked the uploaded information.

I think you are having constipation. It is usual for patients with constipation to strain a lot and pass only small amounts of stool several times before they feel completely empty.
Because of your young age, I would first try treatment for constipation. Movicol sachets 2-3 a day till you get normal stools, then 1-2 a day for some weeks.
If you do not feel better with this treatment in 2 weeks you may benefit from a colonoscopy, which visualizes the bowel from inside and checks for abnormalities.

Meanwhile, try to get high fiber in the diet, drink plenty of water, be active regularly and go regularly to the toilet. I understand it is difficult to feel you are not able to defecate but it is not recommended that you strain as this may cause hernia and hemorrhoids. Try to make use of any feeling that you need to go to the toilet, do not postpone going if you get the feeling (as much as possible).

I hope this answers your query. I remain at your disposal in case further medical assistance is needed.

Regards,
