Question: 70 year old female in good health, treated for BP, hypothyroid, depression and SVT. Frequent episodes of prolonged, weak urinary stream with foul odor; UTI negative.

Thanks for your query.

You reported that you have frequent episodes of prolonged weak Urinary stream. Assuming that all your medical problems are under good control, it appears that your symptoms related to voiding of urine are most probably due to what is called as 'Post Menopausal Urethral Stenosis'.

After menopause there is atrophy of genital organs like vagina and uterus. Urethral stenosis is part of the same process that results in narrowing of Urethra. This gives rise to symptoms like weak stream, prolonged urination and incomplete urination, etc. This causes infection of the stagnant urine.

Please get following basic tests done under the guidance of your doctor-
1) Routine Urine Test and Urine Culture.
2) Ultra sound scanning of Urinary system to exclude cystitis and changes of obstructive uropathy of bladder.
Please consult qualified Urologist for clinical examination further management. He may do endoscopic examination of Urinary Bladder (cystoscopy) and calibration of Urethra followed by long term catheterization.
