Question : Two nights ago I was awakened by sharp pain in my lower abdomen. I went to the toilet and had a bowel movement which included a lot of blood, not in stool, but just blood. Pain slowly dissipated and I have been pain free since. I have not had a b/m since. I've been waiting to see result. Urine during period has been dark, though normal this morning. I am in Hawaii, away from home in Southern Calif.

issue with coumadin. need lab tests and further work up.

Detailed Answer:
Hello and welcome to Ask A Doctor service.
I have read your query and here i my advise.

* As per my clinical experience, the GI bleed with dark urine indicate
possible alteration in the coagulation pathway via consumption of
Coumarin derivative in the name of Coumadin which is a blood thinner.

* You have done right to stop further intake of coumadin at present.

* Follow some basic principles to get well soon.

- Lab evaluation with bleeding time, clotting time , coagulation factor
evaluation as level of 2,7,9,10 factors and most important is Prothrombin
time ( PT ) which should be maintained between 1.5 to 2.5 PT / INR as per
the standard international guidelines.
So has to fix an appointment with your family physician on priority basis.

- Continue to drink more liquids and prefer easily digestible food stuffs.
- High fiber diet to prevent hard stool at present.
- Avoid smoking and alcohol if consuming.
