Whan Does Foreskin Retract Past The Glans After A Preputioplasty?

Question: How long after a preputioplasty should be expected for me to be able to retract my foreskin past the glans with ease. I had the procedure about 6 days ago and the foreskin is still tight with no improvement in phimosis. This may be due to swelling? Or did the procedure just not work?? Also, anything I can be doing to improve my outcome and chances for a successful procedure.. Please advise; thanks in advance...

After preputioplasty the procedure should make the opening wide enough for the head of the penis to come out with ease. I would wait about two weeks before we can safely say that the edema after the procedure has subsided and the preputioplasty has failed.

If your penis is still painful then it would be wise not to touch it wait for the edema and healing process to continue. If it is no longer painful and completely healed you may need to slowly retract the foreskin to dilate the opening. It would still be best to go back to your Urologist for him to see the outcome of his procedure and he may have more suggestions to help you and he'll be able to examine you properly.

Thank you very much. I just had some questions regarding your answers. I am also a medical doctor so please feel free to use medical terminology. Also, my urologist has very very little experience in this procedure (I could not find any XXXXXXX docs who have ever even heard of it).

"After preputioplasty the procedure should make the opening wide enough for the head of the penis to come out with ease" ... by this do you mean the head of the penis should come out with ease immediately after the procedure or after the edema resolves. At this point, the head (glans) is not able to come out; I am hoping this is due to edema.

If it is no longer painful and completely healed you may need to slowly retract the foreskin to dilate the opening."... Well it is not painful when I am not manipulating it. The pain subsided very quickly post-op and recovery from pain/bleeding was excellent. However, if I pull back at the foreskin I do feel mild pain at the site of the sutures. What do you think, should I stretch slowly or will I risk tearing the sutures. Is there anything lost by not touching it or not stretching it? In other words, if I let it be, do I decrease the chances of the procedure working? Also, do you advise any steroid creams to loosen up the foreskin as I have read about on the internet.

1.) Whether the glans should come out - it should freely come out after the edema resolves and everything normalizes. If after the edema resolves and there is still phimosis then the procedure might have failed and you may need a 2nd corrective procedure.

2.) how are your suture lines? is it already coaptated and the sutures already dissolved? 6 weeks after the procedure you may start to dilate the opening my gradually retracting the foreskin to expose glans. You may feel a little tightness and mild pain because you are dilating the opening eventually it would open up but this is only for mild phimosis not for severe cases. Yiu may apply steroid cream at the tip of the penis (preputial skin) 6 weeks post-op so we can be sure that we wouldn't impair the healing process.

Last point, I read in a journal before that a significant number of patients who underwent preputioplasty had to undergo a second procedure and almost half of time when asked should have opted for a circumcision instead of preputioplasty as their primary procedure. Sorry for the late reply doc.

thank you sir.. But was slightly confused by some points..

1) So is two weeks enough to decide whether the procedure has failed me and I may need a second procedure. Or should I give it 6 weeks time? At this point, I am Post-op Day 9 so still have some hope. When should I give up hope and realize and accept that the procedure was not successful in curing the phimosis. It's hard to say if edema is significant b/c the edema may be more in the inner layer. I am assuming/hoping that at this point I cannot easily retract the foreskin because of edema.

2) By second procedure, what exactly do you mean? Another preputioplasty? Why would the second procedure be successful if the first was not? Wouldn't I have to worry about scar tissue, etc becoming a problem. I really prefer not to have circumcision. Do you have any sense how successful the second procedure was in correcting the phimosis (in the journal you refer to).

Doc, I know there is limited experience in this topic; especially in the Philippines where most are circumsized. Here in USA, there is very very limited experience, the urologists I spoke with at my hospital have never even heard of the term preputioplasty. Please help, and if necessary and possible I would love if you can consult with some urologists on this site from countries where this problem is more common (European countries/ India/ Australia - where most are not circumsized.

1) Yes after two weeks the edema usually has already subsided and you can tell if the preputioplasty has worked or not. What is limiting you to retarct the foreskin to expose the glans? Is it a thick band in the foreskin or edema in the suture line?

2) For Second procedure i meant getting a full circumcision since having a phimosis is a medical problem and will lead to infection and higher malignancy risks since you will not be able to clean the penis of smegma.

In the end, i would suggest seeing your Urologist and he will assess his work and he may give you more options since he will be able to do a complete physical examination.
