Question : I am 64. My gynecologist has prescribed vagifem 10 micrograms 2x per week for vaginal atrophy and prevention of bladder problems. I still have my uterus. My family physician thinks this is substandard care, because the estrogen is unopposed. any comments?

Thanks for sharing your health concern with us.

With the dosage prescribed, at your age, progesterone is not added because the risk of endometrial hyperplasia is low. I feel that the treatment should suffice.

Other modes of treatment available are- local androgen application, DHEA, selective estrogen receptor modules and tissue specific estrogen complexes and oxytocin.

Non-hormonal methods like use of soybeans and isoflavones, vaginal moisturizers are also available.

Apart from these, exercises of the pelvic floor should be done to maintain the tone.
