Hi,
Fever with cough in this age is mostly due to some viral infection.
Fever may be very high during initial 2-3 days and the it starts declining. Giving paracetamol(10-15 mg/kg/dose maximum 60 mg/kg/day) 4-6 times a day helps in most of the cases. I would request you to check the dose of paracetamol which you are giving. If fever is persisting even after paracetamol, you may give mefenemic acid(brand name meftal p)after consulting your pediatrician. It is more effective in decreasing temperature and can be given along with paracetamol. The dose is 25 mg/kg/day in three equal divided dosage. It should be given at the same time with paracetamol or one can allow a gap of two hours to see the effect of each medication.
Do tepid sponging(sponging of whole body with lukewarm water) as it is very effective way to control the temperature.
For cough giving cough syrup containing combination of Levosalbutamol a bronchodilator which helps in relieving airway spasm with Ambroxol a sputum thinning agent can be given.
Give plenty of fluids to avoid dehydration and it also helps in decreasing temperature.
Watch for any fast breathing, chest wall retraction or noisy breathing and if you notice any please consult your pediatrician immediately.
I won't get overly worried if all these symptoms are not there.
